- fixed the by usage page not loading in some cases.
- remove SUB Help_PreView; appears to be unused.
- fixed downloaded file being incorrectly saved with txt extension.

Cory Smith a2fc776888 F5 will automatically remove "untitled.exe". 
F5 shouldn't be leaving a bunch of "trash" everywhere. More work needs to be done to this, but this is a start.

Cory Smith 6e53ffb602 Initial Help "reset". 
- Removed "wiki" menu items (Help menu).
- Removed "View on Wiki" *button* in Help window.
- Removed "the red X" in Help window (seems out of place).
- Removed "bread crumb navigation".
- Added placeholder "document title" to Help window.

Fellippe Heitor 20cc9808c2 Fix issue of ghost variables between edits ($Debug) 
Removing a variable between $Debug sessions would
cause occasional crashes on some systems. This fix
invokes the variable watch dialog with a special parameter
so it'll regenerate the list between edits.

New item in Options->IDE Colors allows customizing the color
that's used to draw menu items and dialogs (`Color 7`).
